[S] Source of the reference

Dear all:

I am looking for the source of the reference which didn't get listed on the
back of the chapter on the (on-line) manuals from S-PLUS 4.0.

The paragraph that I have questions about is on page 672 on "Guide to statistics"
(S-PLUS 4.0). The headline for that paragraph is "Multiple Events" on Chapter 24:
The Cox Proportional Hazards Model.

This paragraph talked about the example coming from a study of myocardial infarction
patients where one of the events of interest is fatal or non-fatal....I am doing the research
about patients whom have multiple myocaridal infarctions, but I didn't find any reference using
the method that the manuals talked about so far.

If anyone would know where I can find the reference that this paragraph was mentioned, I would
appreciate it very much! By the way, I tried Medline and CIS from the internet/CD Rom, and also called
the S-PLUS technical support people. I can't find the answer from either of them.
